What is an accident or incident, and why should they be investigated?

The term “accident” can be defined as an unplanned event that interrupts the completion of an activity, and that may (or may not) include injury or property damage.

An “incident” usually refers to an unexpected event that did not cause injury or damage this time but had the potential to. “Near miss” or “dangerous occurrence” are also terms for an event that could have caused harm but did not.

Unfortunately accidents and incidents still take place, often with loss of life, catastrophic damage to facilities and equipment, significant harm to the environment, and negative impact to your reputation.  If accidents / incidents are not thoroughly investigated, or investigated by inexperienced people, there is a high likelihood they will repeat themselves. Outcomes of a proper investigation are that causal factors are accurately identified, corrective actions are precisely specified, and actions are appropriately assigned.
